Whether or not you take part is your choice. If you do want to take part now, but change your mind later, you can pull out of the study at any time and do not need to provide any reason.
Before you decide whether you would like to participate, you are welcome to talk about the study with other people, such as family, whānau, friends, or a trusted advisor. Feel free to do this.
If you agree to take part in this study, please complete and sign the online Consent Form.
A copy of both the Participant Information Sheet and your completed Consent Form will automatically be emailed to you for your records once you have submitted your Consent Form.
