Invitation to Participate in Study

To select participants, LSR needs to collect personal information (e.g., name, address, phone number, email address, age, date of birth, ethnicity, race, medical history, current medication, etc.).

This data is used to match you to trials and to invite you to participate.

You are invited to take part in any study.

It sets out why we are doing the study, what your participation would involve, what the benefits and risks to you might be, and what would happen after the study ends.
